Jan Wolff <wolff@…> fixed the PDF, consolidating into a
single file and removing the cutting marks from around the page edges.
Distribute the result as the new canonical electronic copy.
* OSDevWithCVS_3E.pdf: New file from Jan Wolff.
* index.html: Just point to a single PDF file, instead of to packaged
distributions of multiple files.
* OSDevWithCVS_3E/index.html: Note obsolescence.
* OSDevWithCVS_3E/README: Note obsolescence.
The old PDF files have been left for now, so as not to break links too
quickly, and so that George Gambill can continue converting them to
more editable formats one by one.

conversion from PDF to XML, such that the text is easily extractable
now. I don't want to link to this, but it might be handy to have
under version control, so adding it here.
